“Rarely is one party solely to blame for a collision.
“This tragedy highlights the need for all skippers, regardless of experience or knowledge, to be extra vigilant and adhere to collision prevention rules, especially in busy and confined waters.
“It is also a message to large vessels, such as Cook Strait ferries, that only the highest international standard of bridge management is acceptable.
“It’s important to note that, as the report makes clear, a large number of improvements to maritime safety in the Sounds have been made since this tragedy and that more will follow.
“A tragedy of this nature illustrates the importance of all water users giving the safety of themselves and those around them the very highest priority.”
Other key recommendations contained in the report include:
- That the report is considered by all parties involved in completing the Cook Strait Ferry Review and the Marlborough Port and Harbour Safety Code;
- That Santa Regina’s master is censured for bridge-keeping practices that while they had little bearing on the collision, were unsatisfactory;
- That a flashing light be fitted to Picton ferry wharf to warn of departures and that all small vessels be required to radio ahead when approaching Picton at night.
